Availability Groups Fail over Report

we have SQL 2012 availability group 3 node cluster, It has 5 AG groups and two group always be primary on node A and 3 groups always primary on node B, node C is for backups,

Frequently 2 AG groups from node A are failing over to node B and it becomes very slow due lack of resources until I see and balance the AG groups,

How do create a agent job to get a report of AG group info like which group is primary on which node etc?
